just for giggles, i sent this file to my 2nd printer i have setup the same
and its printing.
so something with first print
just no idea what

Have you tried with safe mode on OctoPrint?

So it seems that there is one plugin that is the issue.

i only have 2 installed
the preheat you recommended and filament sensor reloaded

Those usually do not affect the file contents.
But just for curiosity, disable the Filament Sensor Reloaded.
Is there the Gcode Editor?
