I incorrectly reported this as a bug on the github page for octoprint, but now realize that this maybe more is a problem with something in octopi?
Any one that have had similar issues or have any idea on how to fix it or troubleshoot further?

There are times when I need to refresh the page with OctoPrint if the stream appears to be static or not showing. For me, it always clears up the problem. I think it's related to those times when my computer goes to sleep and I wake it up again.
Unfortunately that doesn't help. Only way I have found to get it streaming again is to reboot the Rpi. And then it stops working again as soon as I start a print.
Well, the Raspi3 has four cores and can handle the load whereas the cheaper/earlier models do not. It's good that you started with a fresh OctoPi image just in case any of the drivers were different.
If it were me, I think I'd go to the 0.15.0 version of OctoPi which is now under general release from what I understand.

from another browser tab/window work better?

Thx for the sugestion, But sadly, trying http://octopi.local:8080/?action=stream gives the exact same problem as before.
And you're not in Internet Explorer (v11) are you?

Mainly using chrome, but have tested Microsoft edge as well.
