Pi reporting undervoltage with official PI PSU

Hi, My PI has regularly been reporting Undervoltage in Octoprint, so I decided to go out and buy the official PSU from Raspberry PI. It still shows the issue.

It's connected to a Prusa Mini, so I need to do the break the 5v line hack to solve this?

The octoprint.log does not report that.

There is an occurrence in the systeminfo:

env.plugins.pi_support.throttle_state: 0xD0000

You may check if there is enough cooling.
